wrongly charging of service fee etc every month

Dear Sir,
I am founder member of ubl visa credit card,

for the last seven month there are wrongly sevice fee etc. being charged.
when I ask on phone about the details of charges, it is told that it is a mistake and after few calls every month it was reversed.
