SPELLING PROBLEM

Respected sir i have issued my card but not activated yet coz they write my mother name wrong
As direction i fill my Appendix form but yet i don't receive any info from bnk kindly plz do some notice on my request

I had applied for debit card at kashmore branch 0438 since last three months but still card is not issued by you, and no body told me the reason for it. so it is requested, kindly tell me the reason.
