correction of my form

My name is Abdul Sattar and my wife's name Sughran Bibi R/o mouza Khadali, Teh & Distt. Rahim Yar Khan. My form No.29721620 ID.No.3130321489203 but entry in 29721520 instead of 29721620 which is likely to be corrected. In form 29721520 shows two persons, I am reapted again that my form No. 29721620.So please correct my form. I shall be thankfull to you in this anticipation.
