I Mr. Chander S/O Wibho Mother's Bahoo Resident of Village Mir Manwawr Ali Khan Talpur P.O Khawaja Taluka Jhando Mari District Tando Allahyar Contact No.0345-3777213, do hereby Confirm and declare on oath that I have obtained two CNIC'S bearing number:
1) 41307-2502137-1 and (2) 41307-7367953-7 respectively due to some mistake of facts on my parts which I submit unconditional apology and surrender four CNIC'S bearing No.41307-7367953-7 to NADRA authorities for cancellation/necessray action.
I hereby assure to NADRA authorities that I h no malafide intention is deducted on my part later on.
I also assure that the CNIC being produce for cancellation has not been used in any fo activity be me It will be label for any legal action if use is proved later on
