CNIC RENEWAL FORM

I live in the United States of America. One of my relatives CNIC has expired. He is unable to travel to Pakistan at this time, so he has been contacting our Embassy & Consulates and also trying to contact on Internet to obtain an application form for renewal of CNIC. No success.

There are two questions.
1. Is there any email address of NADRA.
2. Is there any way that CNIC renewal form can be obtained while in US>
