myname is not included in list of FLP.

Dear sir,
i named mumtaz ali s/o saifal narejo , village khanan ncarejo, deh nandho baharo, u/c Agra, tluka gambat and district Khairpur live in above mentioned village which is situated in kacha flooded area. in rececnt food my house gone under flood , so i CNIC NO. should be listed in flood relief programe.
