Still undelivery of Urgent ID card

Respected Sir!
Assalam-o-alaikum.
It is stated your kind honour that I lost my Original ID card during the days when we were IDPs, I again submitted urgent fee in Mardan,N.W.F.P for a new card, now after 10 months i am deprived my card, my ID Card # 15602-2521315-5 and my Form number is MR00506568and Token number is 40.
I beg to stsate that kindly provide some informations to solve this big problem.
