Student Broad band Pacakage

It is submitted/intimated that I was got Student Broadband package(DSL) on 30/9/2009 but my bill has continueslly coming on normal package.Please registered my student package form and amend it accordingly. Thanks. Scan copy of Student Broadband package(DSL) form is enclossed herewith for ready reference. Thanks again.
