Dear Sir,
This is with reference to the above referred subject.
Recently I came to know that there are still some charges out standing in your books against the above referred my Credit Card Account. Attached please find my initial written request dated December 1st, 2010 for the cancellation of the subject card and request for reversal of any fee/charges charged on my account. At that time I confirmed from the help line on the phone and then sent my original card into two pieces for cancellation and closure of account after reversal of any charges charged. Please note that I cancelled it due to no usage and there was zero payable/outstanding in my credit card account.
From the attached sheet which is self explanatory, you will realize that how and when my above referred account was closed, who are the help line people (names mentioned) who handled my account and at what time & date they confirmed the closure of account as well as reversal of any fee and charges. Please note that time and dates are hand written on the attached sheet which you may confirm from your record as all telephonic lines are recorded.
You are once again requested to please reverse any outstanding fee charges imposed on my account which were never there when I closed my account and issue me a clearance letter for zero outstanding. Please also note that Miss Nishat of your help line on December 31st 2010 at 9:45 am on a recorded telephone line confirmed me that all charges / fee were reversed and the account was cancelled plus they dispatched me a confirmation letter as well. Then on January 15th 2011, at 10:55 am, Miss Quratulain of your help line confirmed me of the reversal and zero outstanding. Now I m shocked to hear that still some charges are outstanding, which is highly non professional, non ethical and unfair from a Bank like UBL. I further request you to please look into the matter personally and resolve.

Attached please find card No. 4105 2500 0042 9719 in original, cut into two pieces for your immediate perusal & action. You are requested to please cancel my credit card (4105 2500 0042 9719) as I don’t require it anymore due to non usage, further also reverse the annual fee/charges charged and close the credit card account under intimation to me.
